New Appointment Process

The care process at the Feeding and Swallowing Clinic follows three simple steps.

  1. To begin the appointment process, a feeding history questionnaire form must be completed (DOC). This form provides us with basic biographic information as well as information that will help in the feeding and swallowing evaluation. If you are a provider referring a client, please see our Resources for Professionals page.

    In addition to the Feeding History Questionnaire, a medical clearance form (PDF) is required.

    This is a form that needs to be filled out by your child's primary care physician. The purpose of this form is to ensure that your child's doctor documents that the feeding evaluation is safe to conduct. This form is very important, as we cannot move forward with the evaluation without it.

    These forms can be mailed, emailed, or faxed to our office. After we have received both forms, we can move onto step 2.

  2. Once we have received the feeding history questionnaire form and medical clearance form, we will give you a call to schedule your child’s appointment. Our clinic operates each Thursday, and the sooner we receive your forms, the sooner we can schedule the appointment! After the appointment date and time has been confirmed, we will send you our appointment packet. This packet includes a confirmation of your scheduled appointment along with some more useful information such as details about the clinic and directions to our location. Please review our Forms page to access the documents you are required to fill out and bring in on appointment day.

  3. Finally, our team of clinicians will evaluate your child on the day of the appointment.
