How can I schedule an appointment?

I’m a provider and I believe one of my patients can benefit from an evaluation by the feeding and swallowing clinic. What can I do?
Please check out our Resources for Providers page for information.
What are the next steps after I fill out the Feeding History Questionnaire form?
After we receive your Feeding History Questionnaire, we will send you a packet of information. Along with our pre-clinic letter, you will receive a medical clearance form that must be signed by your child’s primary care physician before you can be scheduled with our clinic. After we have received these completed forms, we will call you to schedule the appointment. For more information, see our New Appointment Process page.
I’m confused about the medical clearance form. Can you help me get the form from my doctor?
Yes, we can work directly with your primary physician’s office to obtain the medical clearance form. Call or email our clinical services coordinator for more information.
How long is the waitlist for an appointment?
Our clinic works on a first come, first serve basis. Our waitlist is generally one to two months. We encourage you to submit your Feeding History Questionnaire as soon as possible so we can schedule your appointment.
We live very far away from your Morgantown center. Do you have any other locations?
Our clinic is physically located in Morgantown, WV. However, we do offer telehealth appointments so your evaluation can be conducted from the comfort of your home. This option is for West Virginia residents only. All you need is a phone, computer, or tablet with an internet connection. Please contact our clinical services coordinator for more information.
In addition, Marshall University does have a Feeding and Swallowing Clinic that may be closer for families who live in the southern parts of West Virginia.
